Ministry of Health gives Mid-West Toronto’s Ontario Health Team application stamp of approval

On November 18, the partner organizations of the Mid-West Toronto region have been approved to become an Ontario Health Team, marking a significant milestone for patients, clients, families and caregivers in downtown Toronto’s west-end.

Breakaway is proud to be one of more than 50 health care and community service providers that have collaborated to form this OHT.  The team is also proud to partner with over 250 primary care providers in our region. Together, over the last 18 months, we have collaborated to begin to reimagine how care will be provided to the people we serve.

This new model of health care brings together organizations in the same geographic region to better connect patients and providers in their community. By bringing together hospitals, family doctors, community health centers, home care, mental health and addictions services, and social services in our region, we are striving to take the burden off our patients and caregivers and create a more seamless health care experience.

Breakaway joins our partners in celebrating the announcement, and we look forward to continued collaboration as we work together to make health care more accessible to the people we serve. The Mid-West Toronto OHT is based on the fundamental belief that if we design a system the works for the most vulnerable in our community, we are creating a system that works for everyone.

At the core of that planning work was having the support of people with lived experience who provided their invaluable perspective every step along the way. We thank the people with lived experience who have guided us to this important milestone, and we look forward to their ongoing involvement in our planning.

As we look ahead to building the foundations of our OHT, the ongoing feedback of our wider community is critically important. To that end, Breakaway invites our clients, donors and partners to visit our website and anonymously provide their thoughts on how we can more effectively provide care.
