SALARY RANGE: $ 53,268.01 – $64,942.54

POSITION SUMMARY:

 

Pieces to Pathways (P2P) at Breakaway Community Services is a peer-led initiative creating Canada’s first substance use support program for LGBTTQQ2SIA youth 16 to 29 years old in Toronto. P2P provides counselling, case management and group-based supports to youth on an ongoing basis.

P2P offers weekly community dinner in a drop-in format for LGBTQQ2SIA youth that are seeking support with their substance use. P2P also runs a virtual trauma group called Blueprint, which works from a psycho-educational format. The Blueprint group has a level 101 and a level 102.  P2P As well, P2P has a trainee program for LGBTQQ2SIA youth who use/have used drugs who want to learn about working in the harm reduction field.

The Client Support Worker provides case management services, conducts outreach, facilitates groups, and coordinates the community kitchen drop-in space. They work directly with participants by providing individual support to youth with a range of substance use patterns and recovery goals. They are responsible for supporting the community dinner lead in all aspects of operational tasks. They share skills, knowledge, and experience about developing and maintaining substance use goals, coping with triggers and managing stress.

 This is a hybrid role, and requires the successful candidate to work on-site a minimum of 2 days a week.  

ESSENTIAL DUTIES AND RESPONSIBILITIES:

 

  • Conduct intakes and assessments with program participants.
  • Provide one-to-one case management support to between 12-18 youth, in person and via phone calls, virtually and/or text messaging, including facilitating referrals, systems advocacy and navigation.
  • Plan, prepare and cook healthy meals that meet dietary needs of participants
  • Develop and deliver workshops on a range of topics relating to addiction, substance use, mental health, LGBTTQQI2A identities and life skills.
  • Facilitate substance use support programming at the community kitchen once per week.
  • Establish and/or maintain partnerships and/or outreach opportunities with community agencies in alignment with community needs
  • Connect participants with community supports.
  • Attend appointments with participants as needed
  • Support participants to explore. envision and set goals relating to substance use
  • Use lived experience to build relationships with participants in program space and in the community.
  • Maintain accurate clinical records , including up-to-date client information, in accordance with best practices in Catalyst.
  • Adhere to all agency and program policies and procedures.
  • Attendance in all core trainings provided by Breakaway
  • Provide consultation and training related to harm reduction and substance use to community-based agencies and health care providers
  • Develop and maintain effective relationships with partner agencies

 

QUALIFICATIONS:

  • LGBTQQ2SIA identification and lived experience with substance use and/or recovery
  • B.S.W or 2 years of experience preferably as a case manager or a combined equivalent of education and experience
  • Experience working with people with substance use and mental health challenges.
  • Knowledge of Toronto health, social services, mental health, and drug use resources
  • Experience collaborating with and advocating within the following systems: hospital/medical/treatment settings, child welfare, mental health, social assistance, and criminal justice.
  • A strong understanding of the principles of harm reduction and trauma informed care, with the dignity of the client at the center of all interactions
  • Recently demonstrated knowledge and skills in assessment, engagement, case management, group facilitation and crisis intervention
  • A strong understanding of the principles of harm reduction and trauma informed care, with the dignity of the client at the center of all interactions
  • Ability to work with a variety of client presenting situations, personalities, and levels of ability.
  • Experience with and commitment to harm reduction principles and practices
  • Self-directed, organized, and demonstrates the abilities of a supportive team member
  • Certificate in Crisis Prevention and Intervention, First Aid/CPR and CPI certificate and Training on Naloxone Administration an asset
  • Experience facilitating groups.
  • Proven experience working with people with mental health and substance use concerns.
  • Experience implementing strategies for managing substance use.
  • Knowledge about issues that LGBTTQQI2A youth face in Toronto.
  • Available to work evenings.
