POSITION TITLE: Client Support Worker, Satellite Opiate Treatment Program
STATUS: Full Time 1 year contract with possibility permanent

Breakaway Community Services is a multifunction treatment agency that provides a range of
outpatient, day treatment and outreach programs. We are strongly committed to providing

POSITION SUMMARY:
The Satellite is a community-based opiate addiction treatment program. The Satellite provides
comprehensive services to people who are facing challenges related to their use of opiate
drugs. Services offered include opiate agonist therapy (OAT), counselling, case management,
nursing care and health education, a drop-in resource center, meal program, recreational
activities, space for peer support and community building, and access to resources such as
needle exchange, food bank, and clothing. Satellite takes a long-term, trauma-informed, harm-reduction
focused approach to providing these services.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Contribute to a multidisciplinary team in the delivery of comprehensive opiate services
from a harm reduction framework.
Conduct intakes and assessments.
Work with a caseload of clients to provide supportive counselling and case management
services, including facilitating referrals, systems advocacy and navigation.
Maintain accurate case management records and complete reporting required by
funders and supervisor.
Coordinate harm reduction programming, including providing education, naloxone
training and distribution and the ordering and distribution of harm reduction supplies
for the agency.
Facilitate the ‘drop-in’ space, which includes: maintaining safety through facilitating
group dynamics, diffusing conflicts, responding to unsafe behavior, and crisis
management. And further, developing a culture with the drop-in space that both
promotes and welcome diversity and creates a respectful, calming and safe
environment.
Develop and implement responsive programming.
Oversee monthly food program budget and supplies
Meal preparation and maintaining a clean and safe environment (currently paused due
to COVID).
Coordinate and organize program supplies (food and clothing).
Actively participate in supervision, clinical consultations, and team meetings.
May be required to work flexible shifts as needed.
QUALIFICATIONS:
BA/B.S.W plus 2 years of experience preferably as a case manager or a combined
equivalent of education and experience.
Experience working within a multidisciplinary team considered an asset.
Knowledge of Toronto health, social services, mental health and addiction resources.
Strong understanding of opioid use/dependence, treatment options, and current trends.
Experience collaborating with and advocating within the following systems is considered
as asset: hospital/medical/treatment settings, housing, child welfare, mental health,
social assistance, and criminal justice.
Recently demonstrated knowledge and skills in assessment, engagement, case
management, group facilitation and crisis intervention.
A strong working understanding of the principles of harm reduction and trauma
informed care
Ability to work with a variety of client presenting situations, personalities and levels of
ability.
Experience and commitment to harm reduction.
Self-directed, organized, and demonstrated supportive team member abilities.
Certificate in Crisis Prevention and Intervention or willing to be trained.
Current First Aid/CPR and CPI certificate preferred.
Proof of full COVID-19 vaccination required
