POSITION TITLE: Counsellor, Harm Reduction Counselling and Supports Team

STATUS: Permanent full-time

REPORTS TO:  Clinical Director        HRS/WEEK: 35 hr./wk

POSITION SUMMARY:

The HRCS Team provides outreach and community-based substance use and harm reduction counselling and support services throughout the city of Toronto with people who face barriers in their access to services.

The Harm Reduction Counsellor plays an integral role in the delivery of comprehensive harm reduction services. As a counsellor and case manager, the Harm Reduction Counsellor maintains engagement and service delivery to a caseload of clients engaged in Harm Reduction Counselling/Case Management. The position duties include: Case Management, Individual Counselling, Support Groups, Advocacy and Systems Navigation, and strives to provide holistic health supports.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Deliver responsive, accessible, and flexible counselling service from a harm reduction approach.
  • Ability to complete assessments and treatment planning.
  • Provide counselling and intensive case management services, including facilitating referrals, systems advocacy and navigation.
  • Maintain ongoing case management support in the community through home visits, office visits, community visits and support attending community appointments
  • Assessment risk and develop safety plans with clients.
  • Facilitate group programming.
  • Provide harm reduction and overdose prevention education and responses.
  • Collaborate with other health care/social service providers to coordinate services for clients.
  • Maintain accurate clinical records and complete reporting required by funders and supervisor.
  • Develop and maintain effective partnerships and working relationships with other service providers and organizations.
  • Provide consultation and training to community-based agencies and health care providers.

QUALIFICATIONS:

  • Master of Social Work degree, Registered Psychotherapist, or equivalent education and experience.
  • Three to five years’ experience working with people with substance use and mental health issues. Experience working with specific populations the program serves is considered an asset.
  • Knowledge of Toronto health, social services, mental health and addiction resources.
  • Experience collaborating with and advocating within the following systems is considered an asset: hospital/medical/treatment settings, child welfare, mental health, social assistance, and criminal justice.
  • Demonstrated knowledge and skills in assessment, engagement, counselling, case management, group facilitation and crisis intervention.
  • A strong understanding and commitment to the principles of harm reduction, trauma and concurrent disorders, and ability to apply this knowledge to your work.
  • Ability to work with a variety of client presenting situations, trauma and levels of ability.
  • Ability to work independently in the community.
  • A well-developed understanding of the social determinants of health.
  • Excellent written and verbal communication skills.
  • Self-directed, organized, and demonstrated supportive team member abilities.
  • Current First Aid/CPR and CPI certificate preferred.

Salary scale: $58,479 – $70,151
